Which insurance plans we accept

Please bring your child’s insurance card to each visit and show it to the receptionist at check-in. If one of our doctors is not named as your child’s Primary Care Provider (PCP) with your insurance company, you must call your company before your child’s appointment to change your PCP.

Fees for office services:
  • Copays
  • Services not covered by your insurance company
  • Deductibles
  • Co-insurance
  • Ear piercing, which is a $50 charge not covered by insurance
  • $5 fee for nonpayment at the time of service
  • $35 charge added for all Saturday visits, visits that occur after our posted hours, and all holidays. If you are insured, this charge will be billed to your insurance, but it may not be paid and therefore will be charged to you.
  • $50 charge added for all Sunday visits and emergency visits that are seen without a prior appointment.  If you are insured this charge will be billed to your insurance, but it may not be paid and therefore will be charged to you. 
  • Patients whose insurance we cannot verify at the time of service must leave a valid credit card on file or pay in full
  • Form fees: $5 fee for all forms, $10 if needed within 24 hrs. (fulfillment of request is subject to nurse availability). 
  • $5 or $10 fees for all letters- price depends on complexity of letter requested.
  • Fee of $25 for cancellations with less than 24 hours notice and no shows of sick visits ($35 if the appointment was for a check-up or weekend visit)

  • Empire Blue Cross Blue Shield (Including plans through the health exchange)

  • Great West-Cigna
  • Healthfirst Medicaid and Child Health Plus
  • HIP (including Medicaid and Child Health Plus)  We do not participate with EmblemHealth, Comprehealth, HIP Classic, or any HIP Plans through the Health Exchange 
  • Island Group
  • New York State Medicaid
  • Oxford (Including plans through the health exchange)
  • Tricare
  • United Healthcare Community Plan (Child Health Plus and Medicaid)
  • United Healthcare Commercial (Including their Health Exchange product: United Healthcare Compass)
